Accessing PRS Services
Referral Process and Contacts
Individuals can be referred to PRS in a variety of ways including the following referral sources:
- Mental health provider (i.e., therapist, case manager, psychiatrist, residential counselor, etc.)
- Rehabilitation Counselor from the Department of Rehabilitative Services (DRS)
- Family member or friend
- Client him/herself
Referrals from mental health providers or DRS Counselors should include a PRS written referral form and documentation of diagnosis and medications. For more information regarding our services, please contact:
