Accessing PRS Services

Referral Process and Contacts

Individuals can be referred to PRS in a variety of ways including the following referral sources:

Mental health provider (i.e., therapist, case manager, psychiatrist, residential counselor, etc.)
Rehabilitation Counselor from the Department of Rehabilitative Services (DRS)
Family member or friend
Client him/herself

Referrals from mental health providers or DRS Counselors should include a PRS written referral form and documentation of diagnosis and medications. For more information regarding our services, please contact:

Rates for FY 2011
Services	Fees
Psychosocial Day Program	$131.25 per day
Psychosocial Day Assessment	$315.00 per assessment
Employment Intake Assessment	$157.50 per assessment
Work Adjustment Training (WAT)	$78.75 * per unit (maximum of two units per day)
Employment Services (excluding WAT)	$83.50 per hour
Mental Health Support	$95.50 per hour

*Units are calculated as follows: 2.99 hours or less per day is one unit; 3.00 hours to 6.99 hours per day are two units
